Final Reminder!! ALL required paperwork must be turned into the office by 3:00 p.m. Thursday, August 11th for students to participate in practice for Football, Volleyball, or Cross Country. 6th graders have been approved to participate in JH CROSS COUNTRY and FOOTBALL. If your child wants to participate, they must have all paperwork turned in before he/she will be allowed to practice.
Required Paperwork:
Completed Physical, original (not a copy)
Training Rules - signed by student & a parent/guardian
Student Health Survey/Medical Consent Form
Drug Testing Policy - signed by student & a parent/guardian
MHSA Concussion Form - signed by a student & parent/guardian
Spartan Standards Receipt of Handbook
